Understanding the FUT Technique

Follicular Unit Transplantation, commonly known as the strip method, is one of the most time-tested approaches to permanent hair restoration. During the procedure, a skilled surgeon removes a narrow strip of tissue from the back of your scalp — the area that is naturally most resistant to male pattern baldness. This tissue is then carefully divided into individual follicular units under high-powered microscopes, and each graft is precisely transplanted into the thinning or balding areas of your scalp.

While FUE has gained popularity in recent years, the strip technique remains the gold standard for patients who need the maximum number of grafts in a single session. At Barley Hospital, we have refined the traditional FUT approach with modern trichophytic closure techniques. This advanced method allows hair to grow directly through the scar line, making it virtually unnoticeable compared to older strip methods used by many other clinics worldwide.

The FUT Procedure Step by Step

What to expect on surgery day - a detailed walkthrough

1

Local Anaesthesia

The surgeon carefully numbs the donor area at the back of your head using local anaesthesia for a pain-free procedure

2

Precision Strip Removal

A narrow strip of tissue (1-1.5 cm wide, up to 30 cm long) is surgically removed from the donor area

3

Trichophytic Closure

The incision is closed using our advanced suturing technique so hair grows naturally through the scar

4

Microscopic Dissection

Our technical team meticulously separates the strip into individual follicular units under high-resolution stereo microscopes

5

Recipient Site Creation

Your surgeon creates tiny recipient sites in the areas that need coverage at precise angles and depths

6

Graft Implantation

Each graft is placed to match your natural hair growth pattern. The entire procedure takes 4-8 hours

FUT vs FUE: Choosing the Right Method

Understanding the key differences to make an informed decision

FUT (Strip Method)

  • 3,000-5,000+ grafts per session
  • Lower cost per graft
  • Single linear scar (hidden by hair)
  • 4-6 hour procedure
  • Desk work: 3-5 days recovery
  • Best for advanced hair loss

FUE (Individual Extraction)

  • 2,500-4,000 grafts per session
  • Higher cost per graft
  • Tiny dot scars (virtually invisible)
  • 6-8 hour procedure
  • Desk work: 1-3 days recovery
  • Ideal for short hairstyles

When FUT Is the Best Option for You

Three key scenarios where FUT delivers superior results

Advanced Hair Loss

If you are at Norwood stage 6 or 7 and need over 3,500 grafts in one session, FUT provides the coverage you need when other techniques simply cannot deliver enough grafts.

Higher Total Graft Count

Across multiple sessions, FUT can often provide more total grafts than FUE due to the way it preserves the donor area, making it the best choice for full-scalp restoration cases.

More Affordable Option

With a lower cost per graft than FUE, FUT delivers extensive coverage at a significantly reduced price — especially when you choose to have your procedure performed at our clinics in China.

What Our Patients Say

Real stories from real patients who chose FUT at Barley

I was honestly terrified about the linear scar healing after FUT — I'd seen some horror stories online. But Barley's trichophytic closure technique is a game changer. They harvested a 24cm strip for my 3,900 grafts, and the way they stitched it with the hair-bearing edges overlapping means my scar grew hair through it. At month 4 the scar was pink and visible only if I pulled my hair back; by month 9 it's faded to a pale thin line that I can't even find without parting my hair meticulously. Honestly way better than I expected.

Barley uses non-absorbable sutures for FUT that get removed on day 14 post-op — I asked a lot of questions about this beforehand. I was super nervous that having stitches removed would hurt, but the nurse snipped each one gently and I barely felt anything — just tiny tugs. The whole removal took about 15 minutes and I walked out feeling way more comfortable. After the stitches came out, the tightness I'd felt for two weeks almost completely vanished within 48 hours. The timing was perfect and way less scary than I'd built it up to be.

My barber actually commented on how much denser my hair looks compared to his other clients who had FUE. The reason is simple: with FUT strip harvesting, every single follicle from the strip is intact — there's no accidental transection during individual punch extraction like sometimes happens with FUE. I had 3,600 grafts placed across my Norwood 4 top, and when I run my fingers through it I can actually feel the hair shafts are thicker at the root. It's not just visual — the actual tactile density is noticeably better than the patchy FUE result my brother got elsewhere. Night and day difference.

I'm a solid Norwood 6 — pretty much bald on top with only a thin horseshoe left around the sides. Three clinics told me FUE would take 3 separate sessions just to get half coverage, and the cost was insane. Barley told me FUT could harvest 4,400 grafts in ONE session from a single 1.3cm wide strip, and that's exactly what they did. At month 8, I have full coverage from hairline all the way back to the crown swirl. This would have literally been impossible with FUE alone for my grade of baldness. FUT genuinely saved me.

The donor area numbness after FUT was weird but temporary, and it was actually less than I expected. For the first 3 weeks, the strip area felt like when your leg falls asleep — tingly and numb when I touched it. I could still feel pressure and pain if I pressed hard, so it wasn't total loss of sensation. Then week by week the feeling came back, starting from the edges of the scar moving inward. By month 5 I'd say 95% of the normal sensation is back. The surgeon told me some patients have a tiny patch of residual numbness forever, but I didn't — and honestly even if I did, it's on the back of my head where I never notice.

Before choosing FUT I kept asking if the linear scar would be visible at my usual haircut length — I keep my sides at a #3 clipper guard (about 3mm). The team showed me actual patient photos of healed scars with 3mm hair, and I was sold. Eight months post-op I went to get my usual cut and asked the barber specifically to look for a scar on the back. He parted my hair with a comb for a full minute and said he honestly couldn't find anything. The trichophytic closure makes the hair grow straight through the scar line, so at 3mm it's genuinely invisible. I couldn't be happier.

Frequently Asked Questions

Answers to the questions patients ask us most often

How wide is the donor strip typically harvested in FUT surgery
The strip width varies based on your scalp elasticity (laxity) and donor hair density, but at Barley we typically harvest strips between 0.8mm and 1.5mm wide — most commonly around 1.0mm to 1.2mm for the average patient. Wider strips up to 1.8mm are possible for patients with very loose, elastic scalps, but we never exceed what your scalp can comfortably close without excessive tension. The length of the strip is what really varies more: anywhere from 15cm for smaller cases up to 28cm for patients needing 4,000+ grafts. Our surgeons physically test your scalp laxity during consultation to give you an exact estimate before any commitment.
Are absorbable or non-absorbable sutures used for closing the FUT donor area
At Barley we exclusively use non-absorbable surgical sutures for closing the FUT donor strip. The reason is simple: non-absorbable material maintains consistent, even tension across the entire wound for the full 14-day healing period, which minimizes stretching and gives you the thinnest, flattest scar possible. Absorbable sutures dissolve gradually starting around day 7 — precisely when the healing wound still needs strong support. We schedule a dedicated suture-removal appointment on day 14, which takes about 15 minutes and is included in your procedure cost with no extra charges.
Is suture removal painful after FUT surgery
Most patients describe suture removal as mildly uncomfortable rather than actually painful — think tiny pinches and tugs rather than sharp pain. Our nurses use very fine, sharp micro-suture scissors to snip each stitch cleanly, then gently lift and pull each one out. There's no cutting into tissue at this point; everything is just snipping and extracting the suture material. The whole process typically takes 10 to 20 minutes depending on how many stitches were placed. If you're particularly sensitive, we can apply a topical numbing spray 5 minutes before starting — just ask when you arrive for the appointment.
How long does the redness phase of the FUT scar typically last
The scar goes through predictable color phases after suture removal. During weeks 2-4 the scar is usually pinkish-red and slightly raised — this is normal inflammatory healing. From weeks 4-8 it fades to a lighter pink or reddish-purple and flattens considerably. By month 3-4 most patients see the scar turn to a pale pink, and by months 6-9 it typically matures to a pale silvery-white or light skin-tone line that's very easy to hide under hair. Patients with darker complexions may have a slightly longer hyperpigmentation phase (up to 12 months), but this gradually resolves and we can recommend topical creams if needed.
Can folliculitis or pimples develop along the FUT donor scar
Mild folliculitis — small red bumps or pustules around the scar hair follicles — is fairly common during the first 2-3 months as the hair regrows through the healing skin. It's similar to razor burn or ingrown hairs and is not a sign of infection. Typically it resolves on its own within a week or two; we provide a gentle antibacterial shampoo and topical cream in your aftercare kit that helps clear it faster. Rarely, if you get a persistent painful lump or spreading redness that lasts more than 3 days, contact us — that could indicate a true infection that may need a short course of oral antibiotics.
Does hot weather or heavy sweating affect FUT donor healing
Sweating itself doesn't damage the sutures or the healing scar, but excessive trapped sweat combined with bacteria does increase the small risk of folliculitis or surface irritation. If you're recovering in a very hot, humid climate, shampoo the donor area daily starting day 3 with the provided gentle cleanser, and pat — don't rub — the scar dry afterward. Avoid tight hats or helmets pressed against the back of your head for the first 3 weeks, as trapped heat and sweat under pressure is the worst combination. Saunas, steam rooms, and hot yoga should be avoided for 4 full weeks; dry-heat environments are fine once the stitches are out at day 14.
Can I still have FUE extractions after previously undergoing FUT
Absolutely — FUE after FUT is actually a very common combination approach and something our surgeons plan for regularly. After your FUT strip heals, there's still plenty of healthy donor hair both above and below the linear scar line that is perfectly suitable for FUE micro-extraction. Many patients use this two-method strategy: first a large FUT session for bulk coverage (3,500-4,500 grafts), then 6-12 months later a smaller FUE session to add density in specific zones like the hairline or crown. We just need to wait a minimum of 6 months after FUT to let the scalp elasticity fully recover before doing any FUE extractions nearby.
