Understanding Beard Transplantation

Before vs. After — what changes with a beard transplant

Before: Common Beard Problems

Many men struggle with genetics that make it impossible to grow a full, even beard. Whether you have patchy cheeks, a thin mustache, sparse sideburns, or simply can't grow facial hair at all, these concerns affect confidence and self-image.

After: Beard Transplant Solution

The transplanted hair is your own — it responds to shaving, grows at a natural rate, and blends seamlessly with any existing facial hair. Beard hair grows at different angles than scalp hair, and our surgeons replicate these natural patterns precisely.

Who Benefits from Beard Transplant

Patchy or Uneven Beard Growth

If genetics left you with hair in some spots but not others, our transplant fills in the gaps for a uniform, full-looking beard across your entire face.

Inability to Grow Facial Hair

Some men simply can't grow a beard no matter what they try. Our procedure creates the facial hair your genetics didn't provide — permanently.

Scar Coverage in the Beard Area

Whether from accidents, surgery, acne, or burns, we can transplant healthy follicles directly into scar tissue to restore a seamless beard line.

Beard Shape & Line Enhancement

Want sharper cheek lines, a fuller goatee, or more defined sideburns We can sculpt and refine your existing beard to match the style you've always wanted.

Our Beard Transplant Technique

Where artistry meets precision — step by step

1

Custom Beard Design

We work closely with you to design your ideal beard shape, marking exact lines and boundaries directly on your face. Whether full beard, goatee, or Van Dyke — every detail is planned out.

2

Strategic Graft Selection

We carefully select the thickest, most robust follicles from your donor area. These grafts produce the coarse, dense hair that gives a natural beard its masculine character.

3

Multi-Directional Placement

Facial hair grows in multiple directions — mustache downward, cheek hair outward, chin straight down, neck upward. Our surgeons replicate each directional pattern with meticulous precision.

4

Density Optimization

We place grafts at calculated densities that mimic natural beard growth — denser around mustache and chin, slightly lighter on cheeks — creating an authentic, naturally developed appearance.

Beard Styles We Create

Custom-designed facial hair tailored to your face shape and personal style

Full Beard

Complete facial hair coverage from sideburns to neckline. Our most requested option — delivers a bold, masculine look with full cheeks, mustache, and chin.

Goatee & Van Dyke

Focused hair on the chin and mustache with clean-shaven cheeks. A refined, distinguished style that requires fewer grafts than a full beard.

Mustache Only

A thick, well-defined mustache for men who prefer facial hair limited to the upper lip area. Typically requires 400-800 grafts.

Sideburns & Cheek Lines

Define your sideburns and create sharp, clean cheek lines for a polished, groomed appearance. Perfect for subtle facial hair enhancement.

What Sets Our Beard Transplants Apart

The reasons men from around the world choose Barley for their beard restoration

Shave, Trim & Style Just Like Natural Beard

Because the transplanted hair is your own, it behaves exactly like natural facial hair. Shave it clean, grow it out, trim it to your preferred length, apply beard oil, or shape it with a comb — the hair grows continuously just like a real beard and requires the same regular grooming. That's exactly what you want from an authentic result.

Mastery of Multi-Directional Growth

Facial hair growth is far more complex than scalp hair. The mustache grows downward, cheek hair angles outward, chin hair drops straight down, and neck hair grows upward. Our surgeons replicate each of these directional patterns with precision, creating a beard that grows exactly as nature intended — never looking obviously transplanted.

Thick, Masculine Hair Texture

We specifically select the thickest, most robust follicles from your donor area so your transplanted beard has the coarse, dense characteristics of natural facial hair. This careful graft selection is what gives your new beard its authentic masculine look and feel — even to the touch.

What Our Patients Say

Hear from our satisfied patients around the world

I'm half-Lebanese, but I inherited my mother's side of the family — basically zero facial hair density. My cousins all have thick full beards and I always felt left out. Barley designed a custom full-beard coverage pattern for me with 2,600 grafts from the back of my scalp, plus 300 more from my chest since they needed extra density at the jaw. At 13 months, my mustache connects perfectly to my goatee and the cheek coverage is solid. I'm actually growing my first real beard. Feels incredible.

OM

Omar Mansour

Canada

Wanted a Van Dyke — just mustache and goatee, no cheeks. My problem was the mustache was thin on the sides and my goatee had a bald spot right in the middle from an old football injury. Barley placed 900 grafts total: 450 in the mustache to fill out the corners, 350 in the goatee to cover the scar, and 100 to sharpen the chin line. At 10 months, I can style a handlebar mustache if I want. Shaving day 14 was the scariest part, but everything stayed put.

LR

Lucas Ribeiro

Brazil

I'm 36, German, and my beard never grew in evenly — left side was dense, right side looked like someone took a bite out of it. The surgeon explained that uneven beard shape is actually one of the most common problems they fix. 1,400 grafts placed asymmetrically on my weaker right cheek. The stubble grew back at week 4 and it already looked symmetrical. At 9 months I keep a short 3mm beard and nobody notices anything was ever wrong. Perfect five-o'clock-shadow symmetry.

MF

Moritz Fischer

Switzerland

Kenyan guy here — very little body hair and my beard barely existed. Barley harvested 2,000 grafts from the nape of my neck (thicker follicles there) and planted them across my jaw, cheeks, and upper lip. Everyone was worried the hair would come in too soft like scalp hair, but at month 8 it's coarse enough that I need to shave every two days if I want to stay clean-shaven. The 5 o'clock shadow looks completely natural. Going in at day 10 for the first gentle trim felt weird, but the surgeon said it was totally fine.

NO

Njeri Otieno

Japanese, 31. Always wanted a proper mustache — mine grows in two separate patches with nothing in the middle, so I could never rock a pencil mustache or anything connected. Barley placed 600 grafts just in my philtrum and mustache corners. The direction control is what I appreciate most; each hair is angled slightly downward so when I comb my mustache it all lays the same direction. At 11 months I can finally grow a proper chevron. Got stopped by a bartender in Osaka last week who asked where I got my mustache genetics.

TS

Takashi Sato

Japan

My biggest worry before the procedure was that the stubble would look weird — like planted hairs pointing the wrong direction. The Barley team showed me during the design phase how they angle cheek hairs slightly outward, chin hairs straight down, and neck hair upward toward the chin. The attention to detail is insane. 2,100 grafts for a full beard coverage and at 12 months, when I keep it at a 1mm buzz, you cannot tell the difference between native and transplanted follicles. Shaved with a razor for the first time at week 3. Just follow their instructions and you'll be fine.

Frequently Asked Questions

Everything you need to know about beard restoration and facial hair transplant

Where on the body do you typically harvest the donor hair for a beard transplant
The scalp donor area at the back and sides of the head is always our primary and preferred source. The occipital scalp provides follicles with the ideal calibre and permanent growth characteristics. When scalp supply is limited — or when we need extra coarse follicles for dense jaw coverage — we can supplement with beard hair harvested from below the jawline (for patients who already have some) and, more commonly, chest hair. Chest hair is naturally thick and pigmented, which makes it excellent for building a dense-looking lower beard and jawline. All harvesting is done via 0.6mm microneedle FUE.
What graft density is appropriate for a beard transplant, and how many FU per cm² should I expect
Natural facial hair density varies by ethnicity but typically falls between 40 and 70 follicular units per square centimetre. In beard transplantation, we target approximately 35 to 55 FU/cm² — not 100% replication, because scalp-derived grafts have a slightly larger shaft and provide more visual coverage per unit. The mustache and central goatee area receive the highest density (around 50 FU/cm²), while the outer cheeks and upper neck are planted a little lighter (35 to 45 FU/cm²) to maintain a natural gradient.
Is it possible to do a truly full coverage, heavy-duty full-beard transplant including the cheeks and sideburn connection
Yes, full-face beard coverage from sideburn-to-sideburn and down to the neckline is absolutely achievable — but it requires a careful donor assessment and often a two-session plan. A complete heavy full beard on a medium-to-large male face typically takes 2,500 to 4,000 grafts. Patients with excellent scalp donor density and laxity can often complete this in a single long session. Patients with thinner donor supply will do the jaw, mustache, and chin in session one, then top up the cheeks and sideburns in session two approximately 8 to 10 months later.
How soon after a beard transplant can I safely shave with a razor without dislodging the grafts
We ask patients to avoid any direct blade shaving on the transplanted area for a minimum of 14 days. You can use an electric trimmer on its longest guard setting as early as day 7 if you need to tidy up native beard hair around the transplant zone, but do not touch the grafted sites directly. At day 15, most patients can begin using a regular razor with a light, single pass. Avoid pressing hard, going against the grain, or pulling the skin taut until approximately day 21. By week 4, you can shave exactly as you did before surgery.
What causes unnatural stubble direction in a bad beard transplant, and how do you avoid that risk
Unnatural or "pluggy" stubble almost always comes from three mistakes: planting perpendicular to the skin instead of following the natural 25 to 45 degree angle, ignoring the regional direction change across the face, and using grafts that are too large (3-4 hair FU) in the front edge. We prevent this by (1) mapping every direction change on the face with a pen before any anaesthetic, (2) using a custom angled implanter for every region — mustache pointing down, cheek angling slightly outward, chin pointing straight down, neck pointing upward toward the chin, and (3) reserving single-hair grafts exclusively for the perimeter outlines of your beard style.
If donor hair is taken from the chest or other body areas, does the harvesting leave any marks or harm the body long term
No, there is no long-term harm and the cosmetic donor scarring is essentially invisible when we use the 0.6mm microneedle FUE system. The 0.6mm punch creates micro-incisions that heal in roughly 3 to 5 days and leave dot scars smaller than the diameter of a hair shaft — completely undetectable under even 1mm of hair or stubble. Chest-hair harvesting leaves no visible marks because the surrounding native chest hair covers any faint dots. We also stagger harvest sites rather than clearing a single area, so your existing body hair distribution looks entirely natural after full healing.
Will the transplanted mustache and upper lip hair grow too long and require frequent trimming, or does it adapt to facial hair speed
Because the follicles originally came from your scalp, they do keep their scalp growth rate — which means they will grow faster and longer than native facial hair if left completely untrimmed. For most patients this is actually a benefit, because mustache and philtrum hair that grows slowly can be frustrating. In practice, you will simply trim your mustache during your normal beard-grooming routine, just as you would trim a naturally fast-growing mustache. The adaptation period is typically less than a month, and many patients comment that they prefer having slightly faster growth because their new mustache fills into shape sooner.
