Norwood 5 and 6 are the most common advanced baldness patterns we treat. Stage 5 typically requires 3,000 to 4,500 grafts depending on scalp laxity and donor density — this covers the frontal hairline, mid-scalp, and the forward portion of the crown. Stage 6 generally needs 4,000 to 6,000 grafts, usually delivered across two sessions, as the expanded bald crown and vertex dramatically increase the surface area requiring coverage. We calculate these numbers precisely during your consultation using donor density mapping and scalp surface area measurements.
Does the crown or vertex whorl area have worse blood supply, and will that reduce graft survival
The crown vertex does have a slightly different vascular anatomy than the frontal scalp, but this does not meaningfully reduce graft survival when performed properly. The blood supply actually remains robust — all scalp skin receives circulation from multiple arterial networks. Our experienced surgeons avoid placing grafts too deeply into the subcutaneous layer, use proper hydration of recipient sites, and keep the tissue cool during the procedure. These protocols ensure crown survival rates are within 2 to 3 percentage points of the frontal area.
For extensive baldness, is FUE alone sufficient or is FUT strip surgery more appropriate
For advanced Norwood 6 and 7 cases requiring maximum donor yield, both approaches have merits. FUE via our 0.6mm microneedle system allows us to harvest approximately 4,000 to 5,000 scalp grafts from the back and sides while still preserving a natural-looking donor appearance. FUT strip can provide 5,000 to 7,000 grafts in a single session but leaves a linear scar. We recommend FUE for patients who prefer no linear scar or want to wear short hairstyles. For extreme coverage needs, we sometimes combine FUE harvesting with beard and body hair supplementation rather than resorting to FUT strip.
Will my restored hair look like a hairpiece or "hair system" that people can spot
The "hair helmet" or "doll's hair" appearance is the main concern our patients express, and it is something we actively prevent. The key techniques we use are: (1) a feathered, irregular, non-straight front hairline using only single-hair grafts for the first 3 rows, (2) gradient density that builds gradually rather than being uniform, (3) proper angulation that mirrors your original hair direction rather than all grafts pointing straight up, and (4) a natural radial swirl pattern at the crown rather than a flat circle. When these principles are followed correctly, the result is indistinguishable from naturally regrown hair.
Should I aim for maximum density or a natural-looking density for my age
We strongly recommend natural age-appropriate density rather than chasing maximum possible density. A 45-year-old with the density of an 18-year-old actually looks unnatural and draws attention. The average Caucasian scalp has roughly 80 to 100 FU/cm² in youth, decreasing to about 55 to 70 by middle age. Our typical target for baldness restoration is 35 to 50 FU/cm² — a level that reads as "full head of hair" to the human eye while remaining believable for your age. This approach also conserves donor grafts for future touch-ups if your native hair continues thinning.
What is the maximum number of grafts that can be transplanted in one single session
The safe upper limit for a single session depends on your donor density and scalp laxity, but generally we consider 5,000 FUE scalp grafts to be the practical ceiling while maintaining proper follicle hydration, surgical team stamina, and consistent graft quality. For mega-sessions beyond that — up to approximately 6,000 to 7,000 grafts total — we strongly prefer a two-session approach spaced 8 to 12 months apart. This staged protocol protects donor supply integrity, reduces the risk of over-harvesting, and typically produces better overall survival and aesthetic results than attempting everything in one day.
If my scalp donor hair runs out, can additional body hair be safely taken and from where on the body
Yes, body hair transplantation is a well-established supplementary donor source for advanced baldness cases. The beard (chin and jawline area) provides the highest quality secondary donor hairs because they are thick, pigmented, and provide excellent visual coverage per graft — this is our first choice after scalp supply is exhausted. If beard hair is also limited, we can harvest from the chest and, in some cases, the upper back/shoulder area. Leg and arm hairs are typically finer and less ideal for cosmetic coverage, but we evaluate every candidate individually. All body hair is extracted using the same 0.6mm microneedle FUE protocol.
